Last week I was diagnosed with an illness.  I was reading about the medication prescribed to me and left it on the side table in a closed bottle.  My puppy got it the next morning and ate a lethal dose.  My husband discovered what had happened and I raced with her to the Emergency Vet.  They were unable to save her, and we ended up putting her down.  The next step would have been transferring her to another hospital and putting her on a ventilator.  Or manually ventilating her for 24 hours.  Either way it was going to cost a fortune with not guarantee of the outcome. 
I have tortured myself by reading online about dogs who've overdosed with this medicine and have eventually recovered.  I worry we didn't give her enough time. 
My heart is broken.

I am so sorry for your loss. We do what we can with the information we have at the time in consultation with the experts. Second guessing our decisions is normal. Please don’t beat yourself up... Remember the good times and know you did the best you could.
